Why SNF Providers Need to be Concerned about Reducing Hospital Readmissions

Effective October 1, 2012, CMS implemented a Hospital Readmission Reduction Program designed to provide incentives for hospitals to implement strategies to reduce the number of costly and unnecessary hospital readmissions.
